This form helps maintain the CRM Committee Standards for the accurate reporting of volunteer metrics.

Please provide all of the information requested in order to ensure a quick turn-around time. Note: while IDs are not required, it will greatly reduce the turnaround time. Expected turnaround time of a submission with IDs is approximately 5 business days, without IDs it is approximately 10 business days.
